Panchakarma in Kerala: What Actually Happens on Each of the 7 Days

Most Panchakarma explainers describe the treatments. Very few describe the experience of time passing differently when you're inside one. Here is the honest account — day by day — of what a properly conducted 7-day Panchakarma protocol in Kerala actually feels like.

Day 1–2: Preparation (Purvakarma)

The physician does not start treating you on day one. They start reading you. Your prakriti assessment takes 40–60 minutes. Pulse diagnosis, tongue examination, skin tone, eye clarity, the way you answer questions about your digestion and sleep. Your protocol is written after this. On day one you receive Abhyanga — a full-body synchronized oil massage by two therapists — and begin a simplified diet. You feel pampered. You do not yet feel transformed.

Day 3–4: The Oleation Phase Intensifies

Internal oleation begins — medicated ghee taken on an empty stomach before dawn. The quantity increases each day. Your body starts releasing stored toxins (ama) from deep tissue into the digestive tract. Day four is commonly reported as the hardest day: fatigue, mild nausea, emotional unpredictability. This is not a malfunction. It is the protocol working. The physician adjusts your ghee quantity based on how your body is responding.

Day 5: Virechana — The Purgation Day

The central therapeutic event of classical Panchakarma. A medicated purgation administered by the physician in the morning. You remain near your room. The elimination process takes most of the day. By evening you feel lighter in a way that is difficult to describe to someone who hasn't experienced it. The diet that follows is the most restricted of the week — thin rice gruel, no spices, no stimulation.

Day 6–7: Paschatkarma — The Recovery and Reset

The body is now in its most receptive state. Treatments on these days — Shirodhara (warm oil poured in a continuous stream over the forehead), Nasyam (nasal administration of medicated oil), Kizhi (herbal bolus massage) — absorb at a cellular level in a way they could not have on day one. Sleep on night six is commonly reported as the deepest in years. Day seven: the physician does a closing assessment. Diet instructions for the next 30 days are provided. You leave quieter than you arrived.
